Pain of My Heart is the one hundred and thirty-seventh Extra Ops introduced in Metal Slug Attack.

Story

In Ancient Egypt, a priestess named Hathol speaks with HMT about the ritual. HMT asks Hathol if she really wants to go through with this; Hathol, being a curious priestess, insists and the ritual is successfully performed. Prior to the ritual, Hathol pets an accompanying dog.

In the present, Beatriz and Vicky are tasked with bringing in a new Naglfar variant to base, themed appropriately after her. Vicky learns a bit more from her idol, though she is surprised with what Beatriz is telling her.

At the ritual site, Hathol awakens and finds herself in a new form, knowing that the ritual was a success. However, she cannot feel anything in her stone-cold heart, and sets out. She soon confronts the group and severely damages the Naglfar. Vicky suggests to retreat, and Beatriz decides to place the blame on her bodyguards by claiming that they "accidentally" fired a missile during practice; the bodyguards don't even seem to mind at all.

As the Rebels retreat, Hathol watches and the dog from before also appears, now mummified but is still happy to see her. Hathol is still unable to feel any emotions, and continues on her journey to find the Pharaoh, Cleopatra and HMT to find why she cannot feel any emotions.
